We Share Because We Care

 

Through Share2Care medical information about patients that’s needed by health and social care practitioners to support them in their duties will be available to them, in the right place and at the right time. The programme aims to deliver an integrated shared care record for the 4.3million residents living across the North West Coast.

The term ‘medical information’ describes data that is recorded about the health of a patient, such as hospital admissions, treatments and GP attendance, which through Share2Care can be accessed securely by clinical staff working across different organisations involved in the patient’s care, to enable them to deliver safer and more effective care to the patient.
