Health and care staff

Share2Care (S2C) is a service transformation and collaborative digital programme between Cheshire and Merseyside Health and Care Partnership (which is a Sustainability and Transformation Partnerships or STP) and the Lancashire and South Cumbria Integrated Care System (L&SC ICS). The S2C programme empowers health organisations and frontline staff to more effectively utilise the information they have available, in the provision of better health outcomes, for individuals and communities within the North West Coast (NWC).

Share2Care is a Health Information Exchange (HIE) that empowers the authorised Health and Care Professionals that use it, to have controlled access to view certain types of health and care information that is held within different clinical systems across the region – at the click of a button!

You can access the HIE for direct patient care, when you have a legitimate reason for doing so.

The HIE is being rolled out across the North West Coast. It enables frontline staff to view key information about people’s health and care.  You cannot change, edit or download the data, but the HIE allows you to interrogate the data based on a number of categories including date, organisation and speciality.
